Permanent Body Art Facility Information

General information

Facility Requirements

All Permanent Body Art Facilities shall comply with Article 4 starting with Section 119312 of California’s Safe Body Art Law.

Application

To obtain or renew a Permanent Body Art Facility Permit you are required to submit a completed Permanent Body Art Facility Application.  You can download the application forms below.  You can submit the application materials to us in one of three ways:

1. Scan and email to: envhealth@smcgov.org

2. Fax to: (650) 627-8244

3. Mail to: Environmental Health Services, attn: Body Art Program, 2000 Alameda De Las Pulgas, Ste 100, San Mateo, CA 94403

Once the application is complete and all required documents are received, you will be issued a San Mateo County Permanent Facility Health Permit. You are required to prominently display the permit at your facility. Health Permits are valid for one year, and must be renewed annually.
